Sheltered by Grace is a Not for Profit Charitable Organisation with a Statement of Faith in alignment with Luke 4:18 which says “the Spirit of the Lord is upon me, because he hath anointed me to preach the gospel to the poor; he hath sent me to heal the brokenhearted, to preach deliverance to the captives, and recovering of sight to the blind, to set at liberty them that are bruised”.

The Board and Directors of Sheltered by Grace Limited, being of Christian faith will keep this statement in mind at all times when making decisions for the benefit of those we have been called to serve.

 

Role:         

The role of Transformation Workers is to work with clients to ensure access to services and support that assists in restoring health, pursuing education and employment and the regaining of financial independence.

 

Duties:

Clients:

  • Arrange schedules for clients and communicate with Transformation Manager to ensure that appointments are kept
  • Assist clients with day-to-day living, social skills and linkages with government and not for profit services
  • Encourage clients on a daily basis with their social and life skills
  • Assist clients with taking of medication as required in accordance with policies and procedures
  • Ensure confidentiality of information pertaining to clients, team members and Sheltered by Grace

Administration:

  • Maintain accurate paperwork and records as required
  • Report and document all client changes, concerns and incidents in a timely manner to Transformation Manager

Team:

  • Work with the Transformation Manager and other Transformation Workers to improve case management processes and procedures.
  • Network with not for profit providers to link in with service providers.

Quality:

  • Actively participate in, and contribute to workplace quality improvement activities
  • Practice and comply with Sheltered by Grace policies and procedures
  • Comply with all relevant legislation, regulations and professional standards

 

Key Capabilities:

  • Strong negotiation and mediation skills
  • Client liaison and management
  • Ability to follow processes and procedures and suggest improvements
  • Work as a member of a team to share knowledge and experiences

 

Training:

  • Willingness to undertake training as necessary including Certificate III in Mental Health, Community or drug and alcohol dependency
  • Other training as required

 

Essential Requirements:

This role reports directly to the Transformation Manager.  Regular reports on case management issues and clients will be required.
